Gladesville Medical takes your privacy seriously. This page explains in plain language how we collect, use, store and share your personal and health information.

This summary reflects our full Privacy Policy (version 1.1, February 2026), which is the authoritative document and is available at reception on request.

What information we collect

To provide safe and effective care, we collect:

  • Your name, date of birth, address and contact details
  • Your Medicare number, healthcare identifiers and health-fund details
  • Your medical history, medications, allergies, adverse reactions, immunisations, family history and risk factors
  • Clinical notes recorded by your GP during each visit

We collect this information when you register as a patient, during your consultations, and through your interactions with us by phone, email, our website or our online-booking service.

Where relevant we may also receive information from your guardian, other healthcare providers, your health fund, Medicare or the Department of Veterans’ Affairs.

How we use and share your information

We use your information to manage your health and to run the practice — including clinical care, referrals, billing, quality-improvement audits and accreditation.

We will share your information when:

  • It is needed for your care (for example, referral letters to specialists, pathology requests or hospital admissions)
  • You have given us consent
  • We are required or authorised by law (for example, a court subpoena or mandatory disease notification)
  • It is necessary to prevent a serious threat to someone’s life, health or safety
  • You use services such as electronic prescribing or My Health Record

Only practice team members who need to see your information for your care can access it.

We will not share your information with anyone outside Australia without your consent, and we will not use it to market anything to you without your express consent.

Consultation recording

We do not record, duplicate or store audio or video of consultations — including telehealth consultations. If this ever changes in future we will ask for your informed consent first.

CCTV

CCTV cameras are in place in common areas of the practice and outside for security and safety. Cameras are never installed in consulting or treatment rooms. Footage is stored securely.

How we protect your information

  • Electronic records are held in our clinical software on a secure server located on our premises, with individual staff logins and role-based access.
  • Paper records are kept in areas not accessible to the public.
  • Backups run daily, including encrypted offsite copies.
  • All practice team members and contractors sign confidentiality agreements and follow our privacy and information-security policies.

Accessing and correcting your information

You can ask us to see the information we hold about you, or to correct anything that is wrong or out of date. Please speak to our Practice Manager at reception. We will respond to your request within 30 days.

Dealing with us anonymously

You can deal with us anonymously or under a pseudonym unless it is impractical or the law requires us to identify you.
